1. Acceptance of terms
By accessing or using the Rezolv Health website (rezolvhealth.com) or any services provided by Rezolv Health Inc. or its affiliated clinical entities, you agree to be bound by these Terms of Service. If you do not agree, do not use our website or services.
2. Services description
Rezolv Health Inc. is a Management Services Organization (MSO) that supports physician-owned clinical entities providing telehealth-based addiction medicine and mental health services in California, Washington, Nevada, and New Mexico. Our website provides information about these services and allows patients and providers to initiate inquiries and intake.
Clinical services are provided by separately operated, physician-owned clinical entities — not by the MSO directly. These Terms of Service govern your use of the Rezolv Health website and your relationship with Rezolv Health Inc. Clinical services are governed by separate patient agreements and applicable state and federal law.
3. Not a substitute for emergency care
Rezolv Health provides outpatient telehealth services. We are not an emergency service. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
If you are experiencing a mental health crisis or are at risk of harming yourself or others, call or text 988 (Suicide and Crisis Lifeline) or go to your nearest emergency room immediately.
4. Eligibility
To use Rezolv Health clinical services, you must:
- Be 18 years of age or older
- Be a resident of a state in which Rezolv Health operates (California, Washington, Nevada, or New Mexico)
- Have access to the technology required for telehealth visits (internet connection, device with camera and microphone)
- Provide accurate and complete information during the intake process
5. Telehealth consent
By scheduling a telehealth appointment, you consent to receive clinical services via electronic communication. You acknowledge that telehealth has limitations compared to in-person care and that your provider may determine that in-person evaluation or a higher level of care is necessary. Detailed telehealth consent is obtained separately as part of the clinical intake process.
6. Accurate information
You agree to provide accurate, complete, and current information during intake and throughout your care. Providing false or misleading information — including about your identity, location, medical history, or insurance — may result in termination of services and may have legal consequences.
7. Prescription medications
Prescriptions issued through Rezolv Health are for your personal use only. Distributing, selling, or transferring prescription medications to others is illegal. Misrepresenting your condition to obtain prescriptions is illegal and will result in immediate termination of services and may be reported to appropriate authorities.
8. Payment
You agree to pay for services in accordance with the fee schedule applicable to your state and payment method. California services are billed at $249 per month. Other states bill through insurance or at applicable self-pay rates. Outstanding balances may be referred to collections.
9. Intellectual property
All content on rezolvhealth.com — including text, design, graphics, and code — is the property of Rezolv Health Inc. or its licensors. You may not reproduce, distribute, or create derivative works without written permission.
10. Disclaimer of warranties
The Rezolv Health website is provided "as is." We make no warranties, express or implied, about the accuracy, completeness, or fitness for a particular purpose of the information on our website. Nothing in these terms limits your rights under applicable medical malpractice or consumer protection law.
11. Limitation of liability
To the extent permitted by applicable law, Rezolv Health Inc. and its affiliates shall not be liable for indirect, incidental, special, or consequential damages arising from your use of our website. This limitation applies to website use and informational content only — it does not limit liability arising from clinical services, which are governed by applicable medical and malpractice law.
12. Governing law and disputes
These terms are governed by the laws of the State of Delaware, where Rezolv Health Inc. is incorporated, without regard to conflict of law principles. Any dispute arising from these terms shall be resolved in courts of competent jurisdiction in the State of Delaware.
13. Changes to terms
We may update these terms at any time. Material changes will be communicated by posting updated terms on our website with a new version number and effective date. The prior version will be archived. Continued use of our services after changes are posted constitutes acceptance of the updated terms.
