Rezolv Health Inc. is a Management Services Organization incorporated in Delaware, with principal offices at 9000 Crow Canyon Rd Ste S360, Danville, CA 94506. We support clinical practices operating in California, Washington, Nevada, and New Mexico that provide telehealth addiction medicine and mental health services.
References to "Rezolv Health," "we," "us," or "our" in this policy refer to Rezolv Health Inc. and the clinical entities it supports.
