Insurance by state

California — private pay only

Rezolv Health California does not accept any insurance. Treatment is $249 per month — a flat membership fee that covers all telehealth appointments and prescription management. Medication is separate and filled at your pharmacy.

California operates as a private-pay practice by design. No claims are filed, no explanation of benefits is generated, and no diagnostic code appears on a health insurance record. For patients for whom privacy is a priority, this is the point.

HSA and FSA funds are generally eligible for qualified medical expenses including physician visits. Confirm with your plan administrator before applying funds.

Washington — commercial insurance, Apple Health (Medicaid), and self-pay

Rezolv Health Washington accepts:

  • Employer-sponsored commercial insurance plans
  • Private marketplace plans (Washington Healthplanfinder private plans)
  • Washington Apple Health (Medicaid)
  • Self-pay

Not currently accepted in Washington:

  • Medicare
  • Tricare
  • VA / Veterans Affairs
  • Indian Health Service

Nevada — all insurance accepted

Rezolv Health Nevada accepts all insurance, including:

  • Nevada Medicaid (Medicaid Managed Care) — Anthem, Health Plan of Nevada, Hometown Health, SilverSummit
  • Commercial insurance — employer-sponsored and marketplace plans
  • Medicare
  • Self-pay

Nevada Medicaid covers both substance use disorder treatment and mental health medication management. Eligible Medicaid enrollees can access Rezolv Health Nevada services with no out-of-pocket cost for covered services.

New Mexico — Coming Soon

Coming Soon — New Mexico. Rezolv Health is not yet accepting new patients in New Mexico. We are completing credentialing and expect to launch soon. Contact us at 505-210-7924 to be notified.

When we launch in New Mexico, Rezolv Health will accept all insurance, including:

  • New Mexico Medicaid (Centennial Care) — Blue Cross Blue Shield of New Mexico, Molina Healthcare, Presbyterian Health Plan, Western Sky Community Care
  • Commercial insurance — employer-sponsored and marketplace plans
  • Self-pay

Frequently asked questions

Does Rezolv Health accept Medicare? Medicare is accepted in Nevada only. It is not accepted in Washington or California (private pay). New Mexico is coming soon.

Does Rezolv Health accept Medicaid? Medicaid is accepted in Washington (Apple Health) and Nevada (Nevada Medicaid Managed Care). New Mexico Medicaid (Centennial Care) will be accepted when we launch in New Mexico — coming soon. Medicaid is not accepted in California.

Does Rezolv Health accept Tricare? Not currently in any state.

What if my insurance is not listed? Contact us for your state before scheduling. Insurance acceptance is updated regularly as credentialing is completed, particularly in New Mexico where credentialing is ongoing.

Does insurance cover the medication itself? Rezolv Health bills insurance for physician visits and prescription management. Whether your specific medication — buprenorphine, naltrexone, Vivitrol, or psychiatric medications — is covered depends on your pharmacy benefit. Most commercial plans and Medicaid programs cover these medications, but prior authorization may be required for some. Your provider can assist with prior authorization requests.

Can I use an HSA or FSA? HSA and FSA funds are generally eligible for qualified medical expenses. This applies to all states, including California where we do not accept insurance. Confirm with your plan administrator before applying funds.
