What is the Collaborative Care Model?

The Collaborative Care Model is an evidence-based approach to integrating behavioral health into primary care. It involves a care team that includes the primary care provider, a behavioral health care manager embedded in the practice, and a consulting psychiatrist who provides caseload-based consultation — typically without seeing patients directly.

CoCM is one of the most well-studied behavioral health integration models in medicine. Research consistently shows it improves outcomes for depression, anxiety, and other common mental health conditions treated in primary care settings, while reducing total cost of care.

CoCM is also a billable service. CMS and most commercial insurers reimburse for collaborative care services under CPT codes 99492, 99493, and 99494. For eligible practices, CoCM can generate meaningful additional revenue while improving patient outcomes.

Is CoCM right for your practice?

CoCM is generally appropriate for primary care practices that:

  • See a meaningful volume of patients with depression, anxiety, or other common mental health conditions
  • Have or can hire a care manager (social worker, counselor, or nurse) to serve in the behavioral health care manager role
  • Are willing to implement a structured measurement-based care approach (PHQ-9, GAD-7, and similar tools)
  • Want to bill for behavioral health integration services

What is the difference between CoCM and just referring patients to a therapist? In a referral model, your patient goes elsewhere for mental health care — and many never make it to the appointment. In CoCM, behavioral health care is delivered within your practice by a care manager you supervise, with psychiatric consultation available for complex cases. It keeps patients in your care and under your clinical management.

Who provides the psychiatric consultation in CoCM? In the CLARIN Health model, consulting psychiatrists review cases and provide recommendations through the platform — without needing to see the patient directly. This makes CoCM feasible for primary care practices that do not have an embedded psychiatrist.

How does CoCM billing work? CoCM is billed under CPT codes 99492, 99493, and 99494 based on the minutes of care management time per patient per month. Medicare and most commercial insurers cover these codes. The CLARIN team can walk you through the billing specifics for your payer mix.

Do I need to hire new staff to implement CoCM? You need a behavioral health care manager — typically a social worker, licensed counselor, or nurse with behavioral health training — in your practice. Some practices hire this role; others repurpose an existing staff member with appropriate training.

Is there a cost to connect with your CoCM partner? No. We make the introduction at no charge. Any commercial relationship is between your practice and CLARIN Health directly.
