The access problem in psychiatric care

Finding a psychiatrist who is accepting new patients, takes your insurance, and has availability in a reasonable timeframe has become one of the most frustrating experiences in American healthcare. In many parts of Washington and Nevada, the wait for a new psychiatric appointment is measured in months — not weeks. In rural areas, the nearest psychiatric provider may be hours away.

The consequences are real. People who need medication for depression or anxiety go without it. Primary care physicians carry more psychiatric burden than they are trained for. Patients who can afford to pay out of pocket see providers that insurance patients cannot reach. And people who are struggling wait.

Telehealth changes the access equation. Rezolv Health provides psychiatric medication management by telehealth across Washington and Nevada — serving patients in Seattle and Spokane, Reno and Las Vegas, University Place and Carson City, and every smaller community in between. Geography is not a barrier.

Conditions we treat

Rezolv Health providers manage medications for a range of mental health conditions. The following list is illustrative, not exhaustive — if you have a condition not listed here, complete the intake form and your provider will assess whether your situation is appropriate for telehealth medication management.

Depression Major depressive disorder, persistent depressive disorder (dysthymia), and seasonal affective disorder. Treated with SSRIs, SNRIs, bupropion, mirtazapine, and other FDA-approved antidepressants based on your specific presentation and history.

Anxiety disorders Generalized anxiety disorder, social anxiety disorder, panic disorder, and related conditions. Treated with SSRIs, SNRIs, buspirone, and in some cases low-dose benzodiazepines for short-term management under close supervision.

ADHD in adults Attention deficit hyperactivity disorder in adults — including patients diagnosed in childhood who need ongoing medication management and patients newly identified as adults. Treated with stimulant medications (amphetamines, methylphenidate) and non-stimulants (atomoxetine, viloxazine) based on clinical appropriateness.

Bipolar disorder Mood stabilization for bipolar I and bipolar II disorder using mood stabilizers, atypical antipsychotics, and adjunctive medications as clinically indicated. Complex or unstable presentations may require higher-acuity care.

PTSD and trauma-related conditions Medication management for post-traumatic stress disorder using SSRIs, SNRIs, and adjunctive medications. Medication works best in combination with trauma-focused therapy — Rezolv Health can provide medication management and coordinate with your therapist.

What psychiatric medication management involves

Psychiatric medication management is not a one-time prescription. It is an ongoing clinical relationship focused on finding the right medication at the right dose for your specific situation and monitoring your response over time.

Initial evaluation: Your first appointment is a comprehensive psychiatric evaluation by a licensed provider. Before your visit, you complete a detailed intake form covering your symptoms, history, current and prior medications, and what you have tried before. The initial visit typically runs 45 to 60 minutes.

Treatment plan: Based on your evaluation, your provider recommends a medication approach. You discuss the options, potential side effects, expected timeline for effect, and what to watch for. A prescription is sent to your pharmacy if you agree to proceed.

Follow-up monitoring: Psychiatric medications typically take two to six weeks to show their full effect. Follow-up appointments are scheduled to assess your response, manage side effects, and adjust the dose or medication as needed. Once you are stable on a medication, monthly appointments maintain your care.

Medication adjustments: It is common to need dose adjustments or medication changes, particularly early in treatment. Response to psychiatric medication varies significantly between individuals — this is normal and expected, not a sign that treatment is not working.

Medication management and therapy

Psychiatric medication management is distinct from therapy. Medication addresses the neurological and biochemical aspects of a mental health condition. Therapy addresses thought patterns, coping skills, and behavioral factors.

For most conditions, research shows that the combination of medication and therapy produces better outcomes than either alone. Rezolv Health focuses on the medication management side. We maintain referral relationships with licensed therapists and counselors in Washington and Nevada and can help coordinate care if you need both.

Insurance and availability

Washington State: Commercial insurance and Washington Apple Health (Medicaid) accepted. Self-pay available. Medicare not currently accepted. In-person appointments available at the University Place location.

Nevada: All insurance accepted including Medicaid. In-person appointments available at the Reno location.

Frequently asked questions

What is psychiatric medication management? Psychiatric medication management is the clinical process of prescribing, monitoring, and adjusting psychiatric medications over time. It involves an initial evaluation to determine the appropriate medication, ongoing follow-up to assess response and manage side effects, and adjustments as needed. It is an ongoing clinical relationship, not a one-time prescription.

Do I need a referral to see a psychiatric provider at Rezolv Health? No. You can schedule directly through our website without a referral from your primary care physician.

What is the difference between a psychiatrist and a psychiatric nurse practitioner? A psychiatrist is a physician who completed medical school and a psychiatric residency. A psychiatric nurse practitioner (psychiatric NP or PMHNP) is an advanced practice registered nurse with specialized training in psychiatric assessment and medication management. In most states, including Washington and Nevada, psychiatric NPs can independently prescribe psychiatric medications. Rezolv Health providers are qualified to manage the conditions listed on this page.

How quickly can I get an appointment? We maintain availability for new patients and typically offer appointments within days, not months. Same-week appointments are often available.

What if I am already on a medication prescribed by another provider? If you are currently taking a psychiatric medication prescribed by another provider, Rezolv Health can take over your medication management. Your intake form will capture your current medications and we will review your history at the initial evaluation.

What if the first medication does not work? Non-response and partial response to psychiatric medications are common — this is a clinical reality, not a treatment failure. If your initial medication does not produce adequate results, your provider will systematically try alternatives. Many patients require a trial of two or more medications before finding the most effective option.

Is mental health treatment covered by my insurance? Under the Mental Health Parity and Addiction Equity Act, commercial insurance plans are required to cover mental health services at parity with other medical conditions. In Washington, Rezolv Health accepts commercial insurance and Apple Health (Medicaid). In Nevada, we accept all insurance including Medicaid. Call your insurance company before your first visit to confirm your mental health benefits and any applicable deductible.

Can I get controlled substances for ADHD by telehealth? Yes, under current federal regulations, stimulant medications for ADHD can be prescribed via telehealth. Rezolv Health providers follow all applicable state and federal prescribing guidelines for controlled substances.
